DSSF Check Request Form

The foundation is pleased to consider support requests from community organizations serving developmentally disabled individuals or families.

Guidelines

Typical Limit per each request: $100-$500
To be considered for emergency or supportive funding, all requests must be submitted on the DSSF Check Request form and routed and vetted through a community organization.
Forms must be properly filled out and routed via email to the foundation through a community organization. Type written preferred or readable print.
Include information about individual or family needs. It helps the foundation board in making funding decisions when we understand why a client/family needs our financial support.
List percentage of organization’s clients served by Kern Regional Center.

Please note: All Kern Regional Center requests are routed and vetted through Tomas Cubias, Chief Equity Officer.
